Role Overview
The position targets a Staff Engineer who can spot a problem, determine what needs to be done, and drive it to resolution. The role encompasses three critical areas:
1. Real-Time Clinical AI Workflows: Building systems for live clinical audio capture, WebSocket-based transcription streaming, and LLM-powered documentation generation.
2. Agentic Systems: Designing autonomous agents using LLM tool-calling (Amazon Bedrock) with event ingestion and cross-system action capabilities.
3. Dental Data Intelligence: Developing data pipelines that normalize and enrich fragmented practice management system data for analytics.
Key Responsibilities
- Own transcription and LLM summarization pipelines with streaming and structured outputs
- Build agentic systems with tool-calling loops, MCP integrations, and safety guardrails
- Develop full-stack solutions: Node.js/TypeScript APIs, React frontends, PostgreSQL, AWS services
- Design event-driven data pipelines (Kafka, SQS) across external systems
- Apply test-driven development practices for complex, non-deterministic systems
- Operate built systems through monitoring, debugging, cost management, incident response
- Proactively identify and resolve problems independently
- Collaborate on prompt engineering and AI output evaluation
- Use AI assistants daily as core development workflow
Requirements
- 8+ years professional experience
- Strong ownership mindset and responsibility for outcomes
- Ability to ramp quickly in unfamiliar systems independently
- High resourcefulness navigating ambiguity and undocumented systems
- Node.js/TypeScript (async patterns, APIs)
- React (production-grade UIs, real-time experience preferred)
- AWS (Lambda, SQS, S3; Bedrock preferred)
- PostgreSQL (schema design, optimization)
- Event-driven systems experience (Kafka, SQS)
- LLM systems expertise: prompt engineering, structured outputs, tool calling, agent loops
- Data integration experience with unreliable external systems
- Test-driven development proficiency
- Daily use of AI tools (Cursor, Claude, Copilot)
Nice-to-Have
- Healthcare or dental experience
- Speech-to-text systems (Deepgram, Whisper, AWS Transcribe)
- Model Context Protocol (MCP) familiarity
- Multi-tenant SaaS experience
- Infrastructure as Code (Serverless Framework)
- ETL or large-scale data integration background
